Job Description
Seeking a Speech Language Pathologist to provide personalized evaluation and treatment in patients’ homes, focusing on communication, cognition, and swallowing to promote safety and independence.
Conduct assessments using standardized tools to evaluate speech, language, voice, fluency, and swallowing abilities.
Develop and adjust evidence-based treatment plans tailored to patient goals and home environments.
Document progress, interventions, and caregiver education in compliance with regulations and reimbursement standards.
Educate patients and caregivers on communication strategies and safe swallowing techniques within daily routines.
Collaborate with interdisciplinary teams to coordinate care, recommend equipment, and ensure comprehensive support.
Maintain licensure through ongoing education and contribute to team development.
Ideal candidates hold a Master’s degree, CCC or CFY, NYS license, and CPR certification, with preferred experience in dysphagia and neurogenic disorders.
