The Primary Care Provider at the Sokaogon Chippewa Community Health Clinic delivers comprehensive, culturally competent healthcare to community members.
Assess, diagnose, and treat acute and chronic conditions using evidence-based approaches. Provide preventive services, screenings, vaccinations, and health education. Collaborate with a multidisciplinary team to coordinate patient care. Engage with the community through outreach and health promotion activities. Maintain accurate records and ensure compliance with regulations and policies. Stay current with medical standards and participate in professional development. Must hold an active license (MD, DO, APNP, or PA-C) in Wisconsin, with Board Certification or Eligibility, DEA registration, and BLS certification. Preferably experienced in community or tribal health, with strong communication skills and ability to work in a team. This full-time position offers a salary of $94,000-$125,000, requiring in-person work in Crandon, WI, with some travel. Preference is given to Native American applicants per P.L. 93-638.
