Job Description

We are seeking a dedicated Nurse Practitioner to provide high-quality, patient-centered primary care to PACE participants both in the clinic and in their homes.

  • Conduct comprehensive history and physicals, evaluate acute illnesses, and develop individualized care plans.
  • Coordinate referrals, provide preventive care, and manage medication and diagnostic orders.
  • Participate in on-call rotations, home visits, and care management in various settings, including hospitals and nursing homes.
  • Collaborate with the interdisciplinary team and community physicians to ensure seamless care.

    Requirements include current NP registration, AANP or ANCC certification, DEA license, and at least 2 years of relevant experience. Willingness to travel up to 75% is needed.
