Job Description
The Wound Care Nurse Practitioner (NP), supervised by the Director of Nursing, provides specialized wound care in home, nursing home, and assisted living settings.
They assess, develop, and manage individualized wound treatment plans for patients with complex skin conditions. Supervise nurses and medical assistants, educating patients and caregivers, and collaborating with healthcare teams. Requires a Master’s or Doctoral degree in Nursing, current NP license, and wound care certification within 6 months. Minimum 2 years of relevant experience; active patient contact preferred. Must have physical stamina, strong assessment skills, and reliable transportation for travel. This field-based role involves exposure to biohazards, requiring adherence to safety protocols.
