Seeking Board Certified Geriatricians, Internists, and Family Practitioners to provide comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living settings.
Providers will deliver face-to-face primary care, participate in educational sessions, and collaborate with healthcare teams to enhance patient outcomes and facility performance.
