Primary Care Provider

Job Description

We are seeking a dedicated Primary Care Provider to deliver comprehensive, personalized care in multiple Oregon clinics.

Provide in-person, telehealth, and same-day patient services, including assessments, diagnostics, and treatment planning.

Develop individualized plans incorporating lifestyle, supplements, hormone therapies, and prescriptions.

Educate patients on preventive and functional medicine principles to empower healthier lifestyles.

Collaborate with a multidisciplinary team of health coaches, nutritionists, and support staff.

Maintain accurate documentation in the EHR system and participate in team initiatives and quality improvements.

Qualifications include: Active Oregon license (NP, PA-C, MD, DO), board certification, at least 2 years of clinical experience, and strong interest in preventive and functional medicine. Proficiency in electronic health records is preferred.
