Join our expanding healthcare team as a dedicated Nurse Practitioner in our In-Home Palliative Care Program.

Provide in-home assessments and treatment for patients with serious illnesses, collaborating with families and clinical teams to develop personalized care plans.

Support advance care planning and facilitate goals-of-care discussions with compassion.

Serve as a clinical leader to help grow and improve the program.

Advocate for patient needs across care settings.

Requirements: Current California NP license, experience in palliative, hospice, geriatrics, or home health preferred, excellent communication skills, and a proactive, compassionate approach.
