Licensed Clinical Social Worker (LCSW)

Job Description

We are seeking a licensed LCSW with 1-2 years of experience to provide geriatric behavioral health services in long-term care facilities, primarily in Fond Du Lac, WI. This part-time or full-time role involves conducting evaluations, diagnoses, and offering individual, group, and family therapies to nursing home residents.

The position offers daytime hours, flexible scheduling, clinical autonomy, and interdisciplinary team collaboration, with support from a dedicated practice manager. Responsibilities include maintaining Medicare-compliant documentation, ensuring quality care, and managing 10-15 patient contacts daily.

Qualifications include a Master’s degree, full state licensure, proficient communication skills, and technology use. Experience in healthcare settings and Medicare guidelines is preferred.
