Job Description
Seeking Board Certified Geriatricians, Internists, and Family Practitioners to provide comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living facilities.
Responsibilities include delivering face-to-face primary care, participating in educational sessions, and collaborating with healthcare teams. The role aims to improve access, remove barriers, and achieve optimal clinical outcomes across a spectrum of patient needs.
