Community Health is a primary care network offering programs focused on wellness, dental, behavioral health, and pediatrics, serving insured and uninsured patients across multiple locations in Vermont.

About the role:

  • The Licensed Mental Health Clinician (pediatric focus) provides mental health treatment and support to children and their families in primary care settings.
  • Responsibilities include offering emotional support, conducting social needs assessments, developing care plans, and delivering individual, family, and group therapy using evidence-based techniques.
  • Collaborates with a multidisciplinary team, maintains documentation, and stays current with best practices in pediatric social work.

Qualifications:

  • Master’s degree in social work, therapy, or counseling, licensed as LMHC, LADC, LICSW, or LMFT.
  • Experience in pediatric social work and primary care preferred.
  • Strong communication, empathy, problem-solving, and teamwork skills.
