Job Description

Seeking Board Certified Geriatricians, Internists, and Family Practitioners to provide comprehensive on-site primary care in post-acute, rehab, long-term care, and assisted living settings.

The role involves delivering face-to-face care, participating in educational sessions, and collaborating with healthcare teams to enhance patient outcomes and facility performance.
