Primary Care Provider

Job Description

We are seeking a dedicated Primary Care Provider to deliver personalized, integrative healthcare across multiple Oregon locations.

Provide comprehensive patient care through office visits, telehealth, and urgent appointments.

Perform assessments, interpret labs, diagnose conditions, and develop individualized treatment plans including lifestyle, supplements, and hormone therapies.

Educate patients on prevention and functional medicine principles.

Collaborate with a multidisciplinary team of health coaches, nutritionists, and support staff.

Document in EHR and participate in team initiatives and quality improvement.

**Qualifications:** Active Oregon license (NP, PA-C, MD, DO), board certification in Family or Internal Medicine, 2+ years clinical experience, strong communication skills, and proficiency with electronic records.
