Job Description
The Case Management Coordinator collaborates with patients, families, physicians, and the healthcare team to ensure efficient, safe, and cost-effective patient care transitions.
Responsibilities include managing patient flow, discharge planning, and evaluating individual needs to enhance continuity of care, patient satisfaction, safety, and length of stay. They provide support through assessment, planning, implementation, and evaluation, ensuring quality and compliance.
Requirements include a BSN or Master’s in Social Work, valid licensure (RN or LMSW/LISW/LCSW in New Mexico), and preferably case management certification.
Skills needed are effective communication, teamwork, critical thinking, resource utilization, flexibility, multitasking, and organizational abilities.
Experience of at least one year in an acute care setting is preferred. The role involves hospital and office environments, requiring mobility throughout hospital units, good vision, and dexterity.
