Job Description

We are seeking a dedicated PRIMARY CARE PROVIDER to deliver personalized, integrative healthcare across multiple Oregon locations.

Responsibilities include conducting office visits and telehealth consultations, performing assessments, interpreting labs, diagnosing conditions, and developing tailored treatment plans incorporating lifestyle, supplements, and hormone therapies.

Ideal candidates have an active Oregon license as an NP, PA-C, MD, or DO, with board certification in Family or Internal Medicine, and at least 2 years of clinical experience. Passion for preventive and functional medicine, strong communication skills, and comfort in a collaborative, fast-paced environment are essential.
