We are a growing provider delivering innovative medical care in long-term and post-acute settings across the U.S.
Seeking a full-time on-site Nurse Practitioner or Physician Assistant for skilled nursing facilities in Hyde Park, NY. No weekends, nights, or holidays.

  • Conduct comprehensive assessments, develop care plans, and manage chronic and acute conditions.
  • Order tests, prescribe medications, and provide patient and family education.
  • Collaborate with healthcare teams to ensure coordinated care and participate in quality improvement.
  • Maintain timely documentation in electronic records.

    • Requirements include NY licensure, board certification, and geriatric experience preferred. Strong assessment and communication skills, proficiency in EMR, and ability to work independently and travel regionally are essential.
