Job Description

Seeking a compassionate Mental Health Therapist to provide psychiatric medication management and therapy for geriatric patients in nursing home settings.

Deliver individualized assessments and treatment plans.

Provide ongoing psychotherapy addressing emotional and psychological needs.

Collaborate with interdisciplinary teams and facility staff.

Maintain accurate documentation per regulations.

Requirements: Master’s degree in social work or counseling, active IL license, and experience with geriatric mental health.
