Job Description

The Charge Nurse LPN/RN provides personalized nursing care to residents under the supervision of a registered nurse and attending physicians.

  • Utilizes nursing skills to deliver safe, preventative, therapeutic, and rehabilitative care, documenting positive outcomes.
  • Performs delegated tasks following established policies and procedures, administering treatments, medications, and diets accurately.
  • Assesses new residents and develops initial care plans, maintaining accurate charting and narcotic counts.
  • Conducts rounds, monitors residents’ conditions, and reports changes promptly.
  • Supervises CNAs and collaborates with the healthcare team to coordinate resident care.
  • Works in a controlled skilled nursing environment, adhering to infection control protocols.
  • Requires physical activity including lifting up to 50 lbs, standing, walking, and occasional climbing, with specific vision abilities.
  • Must have a high school diploma, be a graduate of an accredited nursing program, and hold valid licensure as an LPN or RN. Current BCLS certification is required.
