Astra Health West is looking for a dedicated *Speech Therapist (ST)* to join our home health team on a *PRN basis*. This is a per-visit position, perfect for clinicians seeking flexibility and rewarding, 1:1 patient care.

*What You’ll Do*

* Provide skilled speech therapy in patients’ homes
* Develop and implement individualized care plans to support communication and swallowing goals
* Document visits accurately and timely in *Axxess EMR*
* Collaborate with the care team (nurses, PT, OT, caregivers, and physicians)
* Educate patients and families to support progress and independence

*Requirements*

* Active Illinois license as a Speech-Language Pathologist (ST/SLP)
* At least 1 year of clinical experience; home health experience strongly preferred
* Reliable transportation for patient home visits
* Strong communication, time management, and documentation skills

Job Type: Part-time

Pay: $80.00 – $125.00 per hour
