We are seeking a dedicated and compassionate Family Medicine APP to join our healthcare team. The ideal candidate will provide comprehensive care to patients of all ages, focusing on preventive care, diagnosis, and treatment of various health conditions. This role requires a strong commitment to patient-centered care and the ability to work collaboratively within a multidisciplinary team.
*Duties*
* Conduct thorough physical examinations and patient assessments to determine health status.
* Diagnose and treat a wide range of medical conditions, including acute and chronic illnesses.
* Provide preventive care services, including immunizations, screenings, and health education..
* Utilize electronic medical records (EMR) systems Epic or for documentation and patient management.
* Collaborate with specialists in areas such as gastroenterology, infectious disease care, and geriatrics for comprehensive patient management.
* Engage in telehealth consultations to enhance access to care for patients.
* Maintain compliance with HIPAA regulations and ensure accurate medical documentation.
*Experience*
Candidates should possess the following skills and experience:
* Proficiency in EMR systems (Epic) and medical coding (ICD-9, ICD-10).
* Background in pediatrics or experience working with children is advantageous.
* Knowledge of behavioral health practices and senior care management is a plus.
* Experience with outpatient services, home care, or hospice & palliative medicine is desirable.

Pay: $100,000.00 – $120,000.00 per year
