*Job Title:* Nurse Practitioner (NP) – Skilled Nursing Facility
*Location:* Johnstown, NY
*Job Type:* Full-Time (Part-Time or Per Diem Options Available)
*Schedule:* Monday–Friday (with some weekend/holiday coverage as needed)
*Reports To:* Medical Director / Facility Administrator
*Position Summary:*
We are seeking a compassionate, skilled, and experienced *Nurse Practitioner (NP)* to join our clinical team at our *Skilled Nursing Facility in Johnstown, NY*. The NP will work collaboratively with physicians, nursing staff, and other healthcare professionals to provide high-quality medical care to our residents, ensuring their physical, emotional, and psychosocial needs are met.
*Key Responsibilities:*
* Conduct comprehensive patient assessments, including history and physical exams
* Develop, implement, and evaluate individualized treatment plans
* Manage acute and chronic medical conditions in collaboration with the attending physician
* Monitor patient progress and adjust treatment as needed
* Provide timely documentation in the Electronic Health Record (EHR)
* Participate in care team meetings and family conferences
* Coordinate care with interdisciplinary teams including nursing, rehabilitation, and social services
* Provide guidance and support to nursing staff regarding clinical concerns
* Order, interpret, and follow up on diagnostic tests
* Prescribe medications and therapies as appropriate
* Ensure compliance with state, federal, and facility regulations
* Participate in quality assurance and performance improvement (QAPI) activities
*Qualifications:*
* Current *Nurse Practitioner license* in the State of New York (required)
* Certification as an Adult-Gerontology or Family Nurse Practitioner (AGNP/FNP)
* DEA registration and prescriptive authority in NY
* BLS certification (ACLS preferred)
* Minimum 1-2 years of experience in geriatrics, long-term care, or a related healthcare setting (preferred)
* Strong clinical assessment and critical thinking skills
* Excellent communication, documentation, and organizational skills
* Ability to work independently and as part of a collaborative team
