Job Description
We are seeking Board Certified Geriatricians, Internists, and Family Practitioners to provide comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living facilities.
Our focus is on delivering high-quality care across various post-acute settings, improving access, and achieving optimal clinical outcomes.
As a team member, you will deliver face-to-face primary care, participate in educational sessions, and collaborate with nurse practitioners and physician assistants.
