Job Description

Join a dedicated healthcare team as a Nurse Practitioner working Monday through Friday from 9am to 5pm in post-acute care settings within your community.

  • Provide high-quality patient care through coordination, communication, and clinical decision-making.
  • Conduct patient rounds, document visits in electronic records, and facilitate necessary tests and procedures.
  • Collaborate with nursing staff, physicians, and leadership to ensure optimal patient outcomes.
  • Maintain accurate documentation for billing and compliance purposes.
  • Possess excellent communication, empathy, organizational, and problem-solving skills.
  • Require Nurse Practitioner license, DEA certification, and relevant experience, preferably in geriatrics, internal medicine, or related fields.
