Job Description

Seeking an experienced Advanced Practice Provider (NP or PA) passionate about compassionate, high-touch wound care in patients’ homes.

This full-time role offers flexible Monday-Friday hours, no on-call, weekends, evenings, or holidays, with daily local travel and reimbursed mileage.

Responsibilities include conducting comprehensive assessments, developing individualized care plans, performing advanced treatments, managing medications, and collaborating with healthcare teams to ensure quality patient care.

Qualifications:

Graduate of an accredited Nurse Practitioner or Physician Assistant program

At least 1 year of clinical experience, wound care preferred

Board Certified/Prepared (AANP, ANCC, NCCPA)

Current unrestricted license in Minnesota and ability to obtain DEA/licensure
