Job Description

Seeking Board Certified Geriatricians, Internists, and Family Practitioners to provide comprehensive on-site medical care in post-acute, rehabilitation, long-term care, and assisted living settings.

Our team cares for a spectrum of patients, from short-term rehab to complex geriatrics in skilled nursing facilities, aiming to improve access, remove barriers, and deliver state-of-the-art care for optimal outcomes.

As a member, you’ll deliver face-to-face primary care, participate in educational sessions, and collaborate with nurse practitioners and physician assistants.
