Job Description

This Nurse Practitioner position offers a starting salary of $100,000 per year and involves providing primary care across multiple sites in South Carolina, including Abbeville, Greenwood, Laurens, McCormick, and Saluda counties.

The role is central to developing and managing a community-based, family-centered primary care program aimed at improving access to high-quality services. Responsibilities include designing an integrated delivery model that emphasizes prevention, health education, and evidence-based chronic disease management, in collaboration with a multidisciplinary team.

The ideal candidate is committed to patient-centered care, with strong communication and interpersonal skills to build relationships with diverse populations. They should possess current knowledge of best practices in primary care and chronic disease management, along with at least two years of experience and active licensure in South Carolina.
