Job Description

Join a dedicated healthcare team as an Advanced Practice Provider, collaborating with Nurse Practitioners and Physician Assistants to deliver exceptional post-acute patient care within the community. This role involves managing and coordinating patient care, conducting rounds, communicating with staff and physicians, documenting visits, and facilitating diagnostic and therapeutic procedures.

We provide comprehensive training, shadowing, and oversight by a Medical Director to support your success. Key responsibilities include effective communication, accurate record-keeping, and adherence to facility policies. Ideal candidates possess strong communication, empathy, organization, and problem-solving skills, with a flexible, team-oriented attitude.

Requirements include at least one year of relevant experience, Nurse Practitioner license, and DEA certification. Preferred backgrounds are in Geriatrics, Internal Medicine, ICU, Emergency Medicine, Hospitalist, or General Practice.
