Job Description

This position involves working over 30 hours per week across multiple primary care sites in South Carolina, primarily focusing on family-centered, community-based care.

The physician will develop and oversee an integrated care model emphasizing prevention, health maintenance, education, and chronic disease management, collaborating with a multidisciplinary team to improve community health outcomes.
