Job Description
The Wound Care Nurse Practitioner (NP) reports to the Director of Nursing and supervises nurses and medical assistants.
Provides specialized wound care to patients in home, nursing home, and assisted living settings, including assessments, care planning, and patient education. Performs advanced wound treatments, monitors healing progress, and initiates appropriate procedures. Collaborates with healthcare team members to coordinate care and ensure optimal outcomes. Supervises and trains nursing staff, maintains detailed documentation, and follows safety and confidentiality protocols. Requires minimum 2 years of NP/medical experience, valid licensure, and certification in wound care within 6 months. Must be physically able to perform fieldwork, including lifting and mobility, with reliable transportation. This full-time role involves travel to various care settings, demanding adaptability, strong assessment skills, and excellent communication.
