Job Description

The Wound Care Nurse Practitioner (NP) reports to the Director of Nursing and supervises nurses and medical assistants.

  • Provides specialized wound care to patients in home, nursing home, and assisted living settings, including assessments, care planning, and patient education.
  • Performs advanced wound treatments, monitors healing progress, and initiates appropriate procedures.
  • Collaborates with healthcare team members to coordinate care and ensure optimal outcomes.
  • Supervises and trains nursing staff, maintains detailed documentation, and follows safety and confidentiality protocols.
  • Requires minimum 2 years of NP/medical experience, valid licensure, and certification in wound care within 6 months.
  • Must be physically able to perform fieldwork, including lifting and mobility, with reliable transportation.
