Job Description

We are seeking a Licensed/Board Certified Nurse Practitioner with wound care experience for a home-based role in a multi-specialty practice.

  • Provide in-person evaluations and management for patients with acute and chronic wounds in home, assisted living, or skilled nursing facilities.
  • Perform medically necessary debridement and procedures following CMS guidelines to promote healing.
  • Follow infection prevention protocols and handle contaminated equipment appropriately.
  • Document services in EMR to ensure proper billing and compliance.
  • Use clinical judgment to assess, diagnose, and prescribe treatments and medications.

    • Requirements include a Master’s Degree, Montana RN license, Nurse Practitioner certification, WOCN or CWCN certification (or within 90 days), minimum 3 years wound care experience, and valid driver’s license.
