Job Description

Are you an Advanced Practice Provider passionate about compassionate, high-touch wound care? This full-time, salaried role offers flexible Monday-Friday hours with no on-call, weekends, evenings, or holidays. You will provide advanced wound care services in patients’ homes, conducting thorough assessments, developing personalized care plans, and utilizing treatments like surgical debridement. The position requires local travel (reimbursed), with equipment and training provided. Qualifications include a nurse practitioner or physician assistant license, at least one year of experience, and wound care knowledge is a plus. The role emphasizes exceptional patient care, collaboration with clinical teams, and proactive disease management. Must be licensed in Minnesota and willing to travel approximately 75% of the time.
