Evaluate and treat speech, language, cognitive-communication, and swallowing disorders in patients at home.

Develop individualized treatment plans and educate patients and families.

Document progress, update care plans, and collaborate with a multidisciplinary team.

Participate in training, conferences, and quality improvement activities.

Ensure compliance with HIPAA and safety protocols.

Requirements include a Master’s in Speech-Language Pathology, state licensure, CCC-SLP, reliable transportation, and preferably prior home health experience.
