Job Description

We are seeking a compassionate Nurse Practitioner (NP) for a Skilled Nursing Facility in Johnstown, NY. This full-time position offers flexible scheduling, including part-time or per diem options, with a typical Monday–Friday schedule and some weekend/holiday coverage.

  • Conduct comprehensive assessments and develop individualized treatment plans.
  • Manage acute and chronic conditions in collaboration with physicians.
  • Order, interpret tests, prescribe medications, and monitor patient progress.
  • Coordinate care with interdisciplinary teams and provide guidance to staff.
  • Ensure documentation compliance and participate in quality improvement activities.

    • Qualifications include NY licensure, AGNP/FNP certification, DEA registration, and 1-2 years of geriatrics experience preferred.
