Job Description
The LCSW-Social Worker for Case Management provides support to patients with complex psychosocial needs, collaborating with the healthcare team and community agencies to ensure safe discharge planning.
Offers a $3,000 sign-on bonus and is a full-time position with a pay range of $40.00 – $48.10 per hour.
Requires a Master’s Degree in Social Work, LCSW license, and current BLS certification.
Preferred candidates have at least one year of supervised experience in a medical setting and clinical expertise.
Responsibilities include comprehensive assessment, crisis intervention, referral, and care management to help patients and families cope with emotional and biopsychosocial issues.
