Growing wound care company seeking an energetic Nurse Practitioner for full-time SNF rounding in Portland, OR.
Manage a full patient load across Skilled Nursing Facilities, assisted living, and home visits.
Provide direct patient care, implement treatment protocols, and ensure accurate documentation.
Collaborate with clinical leadership to improve care quality and compliance.
Maintain certifications, communicate effectively with patients and teams, and follow protocols.
Requires board-certified NP with 1+ year wound care and NP experience, strong communication skills, and willingness to travel.
Preferred certifications include WCC, CWS, or WOCN; must obtain wound care certification within a year if not already certified.
