Job Description
Seeking a compassionate Social Worker to serve patients and families in Salina and surrounding communities.
The role involves providing medical social services aligned with the interdisciplinary Plan of Care.
Assess social and emotional factors within 5 days of admission to evaluate coping capacities.
Assist patients and families in understanding, accepting, and following medical recommendations.
Utilize community resources to improve quality of life within illness limitations.
Document services, observe and report emotional changes, and participate in team planning.
Engage in ongoing education, support staff with loss and family issues, and maintain confidentiality.
Complete all documentation timely and adhere to policies.
Qualifications include a Master’s in social work or equivalent, valid state license (if required), and at least one year of healthcare experience. A commitment to hospice care philosophy is essential.
