Job Description
The Wound Care Nurse Practitioner (NP) reports to the CMO and supervises Nurses & Medical Assistants.
Provides specialized wound care in home, nursing home, and assisted living settings for patients with chronic, pressure, surgical, and complex skin wounds. Performs assessments, develops care plans, educates patients/caregivers, and collaborates with healthcare teams to promote healing. Requires a Master’s or Doctoral degree in Nursing, current NP license, wound care certification within 6 months, and at least 2 years of relevant experience. Must have physical ability for fieldwork, including lifting and mobility, and reliable transportation with a valid driver’s license. Position involves travel, exposure to biohazards, and adherence to safety protocols. Key responsibilities include wound assessment, treatment, supervision of staff, documentation, and coordination of care.
