Job Description

The Wound Care Nurse Practitioner reports to the Director of Nursing and supervises nurses and medical assistants.

  • Provides specialized wound care in home, nursing home, and assisted living settings for chronic wounds, pressure injuries, surgical wounds, and complex skin conditions.
  • Performs assessments, develops care plans, educates patients and caregivers, and collaborates with healthcare teams to promote healing.
  • Requires Master’s or Doctoral degree in Nursing, active NP license, and wound care certification within 6 months. Minimum 2 years of healthcare experience is preferred.
  • Field-based role involving travel, physical effort, and adherence to safety protocols. Must have reliable transportation and a valid driver’s license.
  • Key responsibilities include wound assessment, treatment, supervision of staff, documentation, and coordination of care, ensuring compliance with policies and HIPAA.
