Join a compassionate hospice team as a Hospice Social Worker and make a lasting impact supporting patients and families during end-of-life care.
Conduct psychosocial assessments and develop personalized care plans addressing emotional, social, spiritual, and cultural needs.
Provide crisis intervention, grief counseling, and emotional support to those coping with terminal illness and loss.
Educate families about hospice, advance care planning, and available resources.
Advocate for patient rights and facilitate communication between families, care teams, and community organizations.
Assist with ethical dilemmas, family dynamics, caregiver stress, and resource coordination.
Offer bereavement support and ensure accurate documentation in compliance with regulations.
Participate in team meetings, quality improvement, and adhere to ethical and legal standards.
Qualifications include a Master’s in Social Work, state licensure, and relevant experience. Must have excellent communication skills, reliable transportation, and be able to work in Manhattan, KS. This full-time role offers a starting salary from $68,640 annually.
