Post Acute Nurse Practitioner/ Physician Assistant – Bennington, VT

Job Description

Seeking a full-time or part-time Nurse Practitioner or Physician Assistant to provide primary care in Skilled Nursing and Assisted Living Facilities in Bennington, VT, and surrounding areas.

• Perform primary care visits, medication reconciliations, and annual wellness exams 4-5 days weekly.
• Examine patients, review medical records, order diagnostic tests, diagnose conditions, and prescribe treatments and medications.
• Maintain accurate documentation, educate patients and families, and promote overall health and prevention.
