Job Description

Seeking a licensed social worker for full-time hospice support in the Fruitland, ID area.

• Provide bio-psycho-social assessments, care planning, and coordinate with the interdisciplinary team.
• Offer grief, bereavement, and psychoeducation services, including support groups and counseling.
• Assist with advance directives, crisis management, and community resource referrals.
• Complete documentation, adhere to regulations, and maintain licensure.

Qualifications include a Master’s or Bachelor’s degree in Social Work, current licensure, and willingness to obtain Oregon licensure within 60 days. Hospice experience is preferred.
