Nurse Practitioner – Spanish Required

Job Description

We are seeking a Nurse Practitioner to deliver equitable, value-based primary care to Medicare adults in outpatient settings.

Provide comprehensive primary care, including routine and acute visits, wellness exams, and health assessments.

Coordinate care with specialists, testing facilities, and other providers.

Lead population health initiatives and participate in outreach and education efforts.

Conduct home visits as needed and support care team activities such as phone triage.

The role involves working with a collaborative team, reporting to the Center Medical Director, and focusing on building trusting patient relationships.

Qualifications include a Master’s or Doctorate in Nursing, national certification in relevant specialties, active licenses, and US work authorization. Preferred experience in primary care, geriatrics, or family medicine, with knowledge of HCC coding and health assessments.
