Job Description
The Post-Acute and Long-Term Care (PALTC) Advanced Practitioner (AP) is an APRN or PA working independently in PALTC settings to enhance patient care, improve outcomes, and reduce hospitalizations and costs.
Provide comprehensive assessments, diagnose, treat, and manage acute and chronic conditions, and deliver preventive services.Work independently within scope, making clinical decisions and escalating as needed, while collaborating with facility leadership and interdisciplinary teams.Ensure compliance with regulations, maintain accurate documentation, and participate in quality initiatives.Participate in infection control, health education, and safety protocols.Demonstrate strong communication, leadership, and technology skills, with the ability to work independently and respond swiftly to emergencies.Qualifications include a relevant degree, minimum 2 years of experience (preferably in long-term care), and board certification. The role is full-time, in-person, with competitive benefits and a salary range of $66,948 to $120,000 annually.
