Job Description
The Wound Care Nurse Practitioner reports to the Director of Nursing and supervises nurses and medical assistants.
Provides specialized wound care to patients in home, nursing homes, and assisted living facilities. Performs assessments, develops care plans, educates patients/caregivers, and collaborates with healthcare teams to promote healing. Requires Master’s/Doctoral degree in Nursing, NP licensure, wound care certification within 6 months, and minimum 2 years of clinical experience. Must be physically capable of fieldwork, including lifting and mobility, with reliable transportation. Field-based role involves travel, adherence to safety protocols, and exposure to biohazards. Responsibilities include wound assessment, treatment, supervision of staff, documentation, and coordination of care. Strong assessment, communication, and adaptability skills are essential.
