Job Description

Seeking a Full-Time Nurse Practitioner or Physician Assistant to provide primary care in Assisted Living Facilities in Plymouth, MA, and nearby areas.

• Perform primary care visits, medication reconciliation, and annual wellness exams five weekdays.
• Examine patients, review medical records, order diagnostic tests, diagnose conditions, and prescribe medications.
• Educate patients and families about health, promote wellness, and oversee overall health management.
• Maintain accurate records, inform patients about their health, and stay current with professional knowledge.
