Job Description

Join a dedicated healthcare team providing exceptional post-acute care in community facilities. This role involves managing and coordinating patient care, conducting rounds, documenting visits, and collaborating with staff and physicians.

  • Coordinate and deliver high-quality patient care while maintaining accurate records and facilitating tests and procedures.
  • Communicate effectively with nursing staff and healthcare providers.
  • Conduct patient visits, write orders, and ensure timely documentation.
  • Provide compassionate, patient-centered care, demonstrating strong organizational and problem-solving skills.
  • Require Nurse Practitioner license, DEA certification, and preferred experience in geriatrics, internal medicine, or hospitalist settings.
