Job Description

The Wound Care Nurse Practitioner reports to the Director of Nursing and supervises nurses and medical assistants.

  • Provides specialized wound care in home, nursing home, and assisted living settings, including assessments, care plan development, and patient education.
  • Performs advanced wound treatments, supervises staff, and collaborates with healthcare teams to promote healing.
  • Requires a Master’s or Doctoral degree, active NP license, Wound Care Certification within 6 months, and minimum 2 years of healthcare experience.
  • Must be physically capable of fieldwork, possess reliable transportation, and adhere to safety protocols.
  • Key duties include wound assessment, treatment, documentation, team communication, and training staff, with a focus on patient safety and confidentiality.
