Job Description

We are seeking a compassionate Nurse Practitioner (Family Health) for a full-time, field-based role delivering preventive primary care in patients’ homes, community settings, skilled nursing facilities, and pop-up clinics.

  • Provide comprehensive medical care for adults, women’s health, pediatrics, and geriatrics through in-person assessments.
  • Diagnose, develop treatment plans, and order diagnostics; prescribe medications and perform procedures within scope.
  • Coordinate care with specialists and follow-up post-discharge to reduce readmissions.
  • Maintain accurate electronic records and collaborate with regional colleagues.
  • Travel locally for visits, with mileage reimbursement; work schedule options include standard or 4×10 shifts.

    • Requirements include a Master’s in Family Health, active national certification, state licensure, BLS, and a valid driver’s license. Preferred experience includes 3–5 years in community or home health, managing chronic conditions, and multilingual skills. Compensation starts at $130,000/year, with benefits such as health insurance, 401(k), paid time off, tuition reimbursement, and more.
