Join our compassionate team as a Nurse Practitioner or Physician Assistant providing house call primary care in Pensacola or Milton.
Care for chronically ill, homebound older adults in their homes, focusing on personalized, relationship-based medicine. Conduct about 8 patient visits daily, managing chronic conditions, performing assessments, and coordinating care. Collaborate with a supportive clinical team and maintain thorough electronic documentation. Enjoy a predictable daytime schedule with occasional evening and weekend call (about once every 2-3 months).
