Job Description

Seeking a compassionate Nurse Practitioner or Physician Assistant to provide high-quality preventive care, including Annual Wellness Visits, HCC recapture, and medication reconciliation.


  • Must hold valid CA licenses (Nursing, Nurse Practitioner, PA) and certifications (DEA, BLS, ACLS).
  • Minimum 1 year clinical experience; knowledge of Medicare Advantage and risk adjustment preferred.
  • Responsibilities include conducting AWVs, documenting chronic conditions, patient education, and collaborating with care teams.
  • Proficient in EMR documentation and coding; bilingual skills and mobile care experience are a plus.

    • Offers full staff support, comprehensive benefits, and a full-time role with a salary ranging from $100,000 to $140,000 annually.
