Job Description
The Palliative Care Advanced Practice Provider (APP) is a vital member of the interdisciplinary team delivering goal-aligned care in long-term and skilled nursing facilities.
Conduct comprehensive assessments and manage symptoms such as pain, dyspnea, and distressing conditions in collaboration with physicians. Facilitate goals of care discussions, advance directives, and end-of-life planning with residents and families. Support hospice referrals, transitions, and coordinate with the care team to ensure continuity and quality of care. Provide education to residents, families, and staff on disease progression and comfort strategies. Document visits, contribute to quality improvement, and adhere to regulatory guidelines. Maintain professional development, participate in team meetings, and support facility-based palliative care initiatives.
